This position will support Carilion’s hallmarks of service excellence and quality for all job responsibilities. Will serve as a member of the hospice team by assessing patients’ hospice eligibility, completing hospice admissions, and conducting follow-up visits while also serving as a back-up to the clinical team lead role. Will coordinate care of patient/family unit in collaboration with members of hospice IDT team, Medical Director and attending physician. When serving in the CTL role, is responsible and accountable to lead staff; resolve routine issues, and create a positive, productive team. Refers complex or potentially significant issues to manager/supervisor, including but not limited to; issues regarding compensation, work hours, working conditions, hiring, and formal corrective action. Initiates and facilitates effective open two-way communication and resolution of work site operational issues that support service area goals and objectives. Maintains clinical competencies and provides professional care to patients of all ages in response to agency needs.

Minimum Qualifications Education: Graduate of school of professional nursing.
Experience: For Home Health, one year of experience is required with home health experience preferred.
Licensure, certification, and/or registration: Currently licensed as Registered Nurse in the Commonwealth of Virginia or new graduate of approved program eligible to sit for the Virginia licensure exam. For Home Health, must have valid Virginia Drivers License and pass a Criminal Record Check as per Virginia state law. Must obtain /maintain AHA BLS-HCP.
Other Minimum Qualifications: Must successfully complete orientation and competency requirements. Keyboarding skills required for automated computer systems, including electronic medical records. Must be able to meet essential job functions.
