General Summary/ Overview The Quality and Patient Experience (QPE) department delivers on the Board's vision to advance a culture of treating patients with dignity, respect and equity; within our institutions and in the communities we serve; promote a reliable, consistent, and coordinated care experience across institutions; and achieve the best clinical outcomes possible. The Project Specialist of Social Determinants of Health will manage a program in primary care practices across Mass General Brigham to screen and refer patients for the social determinants of health. Patients identified with an unmet need associated with a determinant of health, such as food, housing, transportation, education, and others, are provided with education or resource connection. The Project Specialist will support workflow implementation and optimization within primary care with the aim to increase screenings and responses for patients who request assistance. This role will facilitate meeting contractual requirement of MassHealth ACO agreement to screen for health-related social needs, with an eye to expand this screening to a broader patient population as part of a rapidly advancing United Against Racism system-wide initiative. The position is funded through December 2022. Principal Duties and Responsibilities • Provide project management for the ongoing work of the social determinants of health program across Mass General Brigham. • Track progress across multiple initiatives, identify and flag bottlenecks and risks. Has ability to manage and prioritize across multiple projects to meet deadlines. • Able to prioritize needs of traditionally underserved populations, elicit and incorporate diverse perspectives. • Support the monitoring and evaluating of the SDOH screening and response intervention. • Prepare presentations, meeting materials, minutes, and action plans. • Partner with analysts in the development of operational dashboard for screenings, referrals, issues and outcomes. • Develop relationships with primary care clinic staff in the design of workflows for screening and response • Support the expansion of screening into additional practices in primary care across Mass General Brigham • Support the design of Epic tools to facilitate referrals to community resource specialists or community health workers • Work closely with Partners providers and other departments within Partners to implement the initiative. • Work closely with technical staff to optimize Epic build to support program enhancements, such as clear visualization in patient chart of screening responses • Work closely with Medicaid ACO team in order to ensure screening meets contract requirements • Coordinate with internal and external community supports for the Medicaid population. Qualifications Qualifications • Bachelor's degree and 4+ years of experience OR Master's degree and 2+ years of experience • Experience in public health, health management, policy, or other related fields strongly preferred Skills/ Abilities/ Competencies • Strong organizational, administrative, and project management skills • Ability to prioritize and resolve critical issues efficiently and effectively • Ability to work with all levels of an organization • Ability to effectively present ideas and information • Strong written and verbal communication skills • Strong commitment to customer service • Ability to work independently • Ability to flex within a dynamic organization/policy environment. • High degree of initiative and enthusiasm for learning new concepts and working with new tools and sources of information. • Demonstrated commitment to health equity, antiracism, social justice causes, or community development.
