Family Medicine at Gabriel Park is a community clinic located in SW Portland. We serve our patients in varying stages of life and throughout their lives. Our services include general family medicine, transgender care, maternity care (including deliveries), pediatric care, behavioral health, sports medicine, travel immunizations, acupuncture, a retail pharmacy, x-ray, minor procedures, and health maintenance with focused preventive care. We are a high volume clinic that sees up to 250 patients and receives 400-600 phone calls daily. We have 39 providers plus 60 staff members. Our clinic guides itself though our vibe: Compassionate, Committed, and Collaborative. The OHSU Clinical registered nurse (RN) provides compassionate, evidence-based, and efficient care to individuals, families, communities and patient populations. The Clinical RN's care delivery is consistent with the Oregon Nurse Practice Act, the ANA Scope and Standards of Practice, and the ANA Code of Ethics. The Clinical RN demonstrates the professional role obligations of scientist, leader, and knowledge transferor [Onsomble Model of the Professional Role]. Professional accountability enriches the Clinical RN's engagement as a leader in promoting an inter-professional culture of collaborative decision-making, innovation, life-long learning, and teamwork. The Clinical RN exemplifies the principles of a Culture of Safety by committing to a Just Culture, a Reporting Culture, a Learning Culture, and an Engaged Informed Culture. The RN will provide quality care management and triage nursing care to patients consistent with OHSU Family Medicine at Gabriel Parks care philosophy: patient-centeredness, advanced access, providing a robust medical home for patients, team-based care and empanelment. The RN will set a positive example of nursing for the Family Medicine residents, staff and visiting students. The RN serves in a clinical leadership role to the Gabriel Park team. The RN ensures that the clinic functions smoothly and efficiently while keeping the patients well-being paramount. The individual represents the patients, providers, staff, the clinic and OHSU. They will conduct themselves in a friendly, courteous and professional manner when involved with any activity associated with OHSU. The presentation of medical care is an important component of the quality of medical care. Clinical Skills and Duties:
Orientation period: 6 weeks Expected abilities of the RN: Ability to perform duties using theory-based nursing practice model, evidenced-based practices, and shared governance in a multidisciplinary team model. Ability to participate in a multidisciplinary clinical team. Knowledge of clinical quality improvement models and process. Knowledge of scope of patient care services provided by other clinical service lines and the referral process. Knowledge of effective teaching processes and principles in planning, providing and evaluating optimum health care. Knowledge of quality management and improvement processes to comply with internal and external standards, i.e., JCAHO. Knowledge of general medical procedures and the ability to anticipate complications and the indicated therapeutic interventions. Knowledge of clinic practices and ability to act as a resource for clinic personnel and assist the providers as necessary. Proficiency in the use of appropriate EMR, computer based software and office equipment. Ability to maintain quality safety and infection control standards. RN must possess the ability to apply the operational principles of the GPFM care model. Knowledge and ability to apply primary care best practices. Ability to mentor and orient new staff members. Accountability: Provides accurate and timely documentation of care of patients in the patient record - EMR. Assure proper transfer of patient care and coverage of duties when leaving the clinic. Initiate appropriate interactions with patients, providers and other staff regarding any relevant patient care or operational issues. Policies and Procedures: Implement agreed upon changes in function, policies and procedures. Establish appropriate patient/clinic flow priorities and follow through to completion. Participate in the formation, development, implementation, and evaluation of health care plans for patients. Other duties/projects which are assigned by supervisor and/or Lead Nurse are to be followed through to completion. Meetings: Attend and participate in relevant departmental, clinic, nursing, and patient care meetings. Assist and actively participate in the ongoing improvement of the clinic.
