The Executive Director will utilize their experience and knowledge to provide strategic oversight, overall program direction and leadership for the hospital case management program which encompasses care coordination, utilization management, and social work. The Executive Director will develop, implement, and sustain a comprehensive Case management Triad Model and other related programs that are consistent with Cedars-Sinai's mission, social work code of ethics, case/utilization management standards, patient care standards, and requirements of regulatory agencies. Ensure effective staff training, monitoring and evaluation. Facilitate best practices related to excess days reduction, efficient patient flow, transitions of care, identification of Social Determinants of Health (SDOH) through appropriate discharge planning for all patients across the continuum and appropriate reimbursement for services provided. The successful candidate will promote the role of case management, social work & utilization management within the health system by serving as an effective liaison with directors, physician leaders, payers, and other stakeholders.
