![]() Patient Navigator Non-Clinical 3 SOM: Pediatrics Sacramento Campus Department Description The Department of Pediatrics in the School of Medicine consists of 14 Divisions and Sections. The overall mission of the Department of Pediatrics is to provide compassionate, state of the art health care for children, to advocate for their benefit and to discover new knowledge in the areas of teaching, research and clinical activities. As part of this mission, the CAARE Diagnostic and Treatment Center provides patient care (mental health services), teaching and research on behalf of abused/neglected and at-risk children. Additionally, CAARE Center staff collaborate with staff at family-serving organizations for children and families experiencing homelessness, with pediatricians and pediatrics staff to provide health and mental health care services to children receiving child welfare services, and those whose families are considered at- risk at the CIRCLE Clinic. Mental health services are provided to children ages 0 to 21 through service contracts and grants in outpatient clinics, pediatric clinics, juvenile hall, home, and community settings. THIS IS NOT AN H1- B OPPORTUNITY Responsibilities The role of this position is to provide executive level input to ensure that families are equal partners in the planning, development, and implementation of services. Using a trauma-informed approach, the Family Navigator assists families with accessing housing support, needed community resources, navigating county systems, providing education related to treatment and resources, and coordinating with patient's community service providers. Identifying patient's preferences and any barriers to the treatment plan implementation. The family navigator will advocate for families and children, communicate with mental health providers, different community agencies, child welfare, and health systems to support the family and prevent fragmentation of care.
