Additional Information About the Role
- Shift information: The Behavioral Health Urgent Care is open Monday-Saturday from 9am to 7pm. The work schedule will include alternating weekend coverage.
- Team Dynamic: This team is embedded in SSM DePaul’s campus, so you will work with a combination of SSM Nurses and Nurse Practitioners, as well as BJC Behavioral Health clinicians and registration staff. You will also work with clients presenting in crisis that arrive at the facility
- Location: SSM DePaul’s Behavioral Health Urgent Care in North County
- MUST HAVE skills for this position: Flexibility, experience with crisis (strongly preferred), organization, teamwork, computer skills, familiarity with community resources.
- Job responsibilities: Providing necessary support to clients in crisis, identifying/providing resources to clients, assisting clinical staff with resource linkage, scheduling screening appointments with community agencies, front desk support as needed.
Overview
BJC Behavioral Health is a community health center that provides and coordinates behavioral health services for more than 8,000 seriously mentally ill adults and seriously emotionally disturbed children in St. Louis City, St. Louis County, St. François, Iron and Washington counties. As an Administrative Agent of the Missouri Department of Mental Health (DMH), BJC Behavioral Health serves as a major point of entry for people eligible for mental health services funded by DMH and is responsible for serving as gatekeeper to the public mental health system. Role Purpose Community Health Worker provides preventive health improvement services by working closely with interdisciplinary care team, internal/external agencies to address social determinants of health to improve the overall health and well-being of the child and family. Responsibilities Work daily as a community liaison to identify gaps in care by assisting clients in accessing the necessary resources to remove barriers categorized as social determinants of health.Provide referrals for appropriate services as needed by assisting in assessments/screenings or conducting follow-up care with patients in person, via phone, home visits and/or visits to other settings where clients are accessible.Establish and maintain trusting relationships with clients on a daily basis to effectively work with an inter-disciplinary team, internal departments, and with community organizations.Document and track ongoing activities, care plans, and results in an effective manner while strictly adhering to the policies and procedures in place.Continuously communicate and expand knowledge of community resources, services and in the hospital or community based agencies the programs provided to reduce cultural and socio-economic barriers between the clients and institutions.BJC has determined this is a safety-sensitive position.
