Additional Information About the Role BJC Hospice of Illinois in the Glen Carbon and the surrounding area is looking for a Hospice Liaison to visit patients and families, physicians' offices, assisted living and nursing home facilities to educate on the benefits of hospice and encourage referrals to BJC Hospice. This is primarily a sales position.
Requirements: Bachelor's Degree At least 1 year of hospice liaison experience with total of 2 years of hospice experience. Valid Driver's license and automobile insurance as the position involves a large amount of driving.
Overview BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.
Hospice, the final stage of BJC's continuum of care, is a special kind of caring for patients with a life-limiting illness. Services are provided in the comfort of the patient's home or skilled nursing facility. The Hospice staff are sensitive to the physical, psychosocial, emotional and spiritual needs of terminally ill adult and pediatric patients and their families. We provide a multi-disciplinary team of healthcare professionals and volunteers, specially trained in symptom management, pain control, counseling and bereavement services for the dying. Our Hospice services include alternative therapies such as music, art and massage therapy. Our Hospice programs provide palliative care by helping patients manage their pain and symptoms while living their lives with daily peace and dignity. Preferred Qualifications Role Purpose Advocate for the Medicare benefit and educate on services available from BJC Hospice, Home Infusion, Home Health, and/or Home Medical Equipment to members of the healthcare and patient community. Develop community relationships and maintain positive relationships with customers and referral sources within the Community. Respond to customer and community needs and describe the Medicare Conditions of Participation to increase awareness of available services and program requirements.
