The West Tennessee Coordinator for Disability Pathfinder will share responsibilities in assisting individuals needing information about community resources and disability-related services as well as support outreach efforts.
100% Remote - must reside in West TN. Job Responsibilities: Provide information and assistance to individuals seeking resources via phone, email, and website chat:Assess, clarify, and determine the needs of each individual seeking information.Based on needs identified, provide individual with community resources, agency services, and disability information needed.Inform individuals of tips for navigating resources - application processes, waiting lists, qualifications, etc.Problem solve and think creatively as resources are limited.Document each case or query in RedCap survey form.Follow-up with individuals needing additional resource information.
Assist with the development, updating, and maintenance of information on Pathfinder's website:Identify and provide changes needed to correct information on the website.Assist with researching and contributing new providers, resources, and events to add to website.Assist Pathfinder Website Coordinator with website listings management.
Community Engagement & TrainingParticipate in selective community meetings to network and identify clients and/or services.Provide training presentations at various local, regional and statewide meetings/conferences.Represent Pathfinder at resource fairs and other events to share information, disseminate materials, network, and answer questions.Contribute relevant information to Pathfinder's social media network, e-newsletter, and website.
Work collaboratively with Pathfinder staff and other Vanderbilt Kennedy Center programs Requirements: Degree in Social Work, Counseling or other related degree preferred and experience working with consumers and family members who have disabilities. Need to have a working knowledge of community resources and disability services available in Tennessee for children and adults with disabilities as well as knowledge of government systems. Applicant must have technical computer skills and organizational skills. Must possess excellent written and oral communication skills, ability to problem-solve; multi-task; and work independently. Proficiency in Word, Excel, Power Point, Teams, and Outlook required.
