JOB SUMMARY The Hospice Social Worker is assigned a group of patients to oversee social work care for both patients and families, and provides patients and family members with the education and advice needed to cope with issues which arise from various emotional or physical problems as outlined by the Texas State Board of Social Worker Examiners scope of practice. Coordination includes a variety of services including crisis intervention, bereavement, client advocacy, referrals, education, discharge planning and implementation, counseling and psychosocial assessments. Location: Temple, TX Setting: Hospice Schedule: FT Monday-Fri ESSENTIAL FUNCTIONS OF THE ROLE - Provides patients and family members with highly complex education and advice needed to cope with issues which arise from various emotional or physical problems as outlined by the Texas State Board of Social Worker Examiners scope of practice.
- Accountable for developing a written plan of care for each patient, family and caregiver before providing care and services. Care coordination among team members is demonstrated through a review of team conference notes, clinical notes, and telephone communication contained in the clinical record.
- Helps a patient and family with finding alternate means for help for basic life needs.
- Conducts patient assessments as required by Medicare Conditions of Participation and documents in the medical record.
- Coordinates patient respite stays at local facilities to include contracting, transportation and clinical staff for safe patient transfers to/from facility.
- Serves as a liaison with local community resources and their staff to deploy a smooth transition and placement of the hospice patients.
- Helps with admissions, discharges, transferring, arranging alternate levels of care, and attending the deaths of the agency patients as needed.
- Participates in the planning and helps the Hospice Annual Memorial service for families of patients who passed away the past 12 months.
- Coordinates with Texas Adult Protective Services when needed to make reports concerning patient safety or to help in Texas APS investigations about patient abuse and safety.
- Performs other position appropriate duties as required in a competent, professional and courteous way.
