Job Summary
The Advanced Practice Nurse, Hospital at Home (APN-HaH) is accountable for providing comprehensive patient care congruent with role and approved population (Family/Individuals across lifespan and Adult/Gerontology) through systemic assessment, diagnosis, treatment, management, and follow up affecting direct patient care under the supervision of HaH Clinical Medical Director. The APN-HaH serves as a clinical expert and consultant to support the healthcare system in providing patient-centered, culturally-competent, outcome-focused therapeutic care within the management of specified populations through independent and collaborative clinical management. The APN-HaH aligns practice with professional rules and regulations, accreditation standards, and organizational policies and procedures.
Clinical Summary:
This APN-HaH provides high-quality patient care to various patient populations within the framework of the Hospital at Home Program model. The APN-HaH will be accountable for planning, implementing, evaluating and communicating all phases of patient's plan of care to the Medical and nursing teams. This APN is responsible for facilitating an environment that supports professional practice and empowers all interdisciplinary healthcare workers to provide safe, effective, compassionate, and efficient patient care.
Minimum Qualifications
Degrees:
a) Bachelor's Degree
b) Master's Degree: Graduation from an accredited School of Nursing with a Master's degree in Nursing with completion of a university-based program for Advanced Practice Nurse required. Acute Care NP or Adult-Gerontology Acute Care NP preferred
Licenses & Certification:
a) Certificate:
Current specialty certification from nationally recognized certifying organizations
Current prescriptive authorization (or eligibility) number from the Texas Board of Nursing (TX BON)
ANCC Certification
Other certificates as required by unit/service.
b) DEA: Current and unrestricted Drug Enforcement Registration (DEA) License (or will be required to obtain have a DEA registration license within 3 months of hire) (Nurse Practitioner);
c) Advanced Practice Registered Nurse: Current recognition as APN with the Texas Board of Nursing (TX BON)
d) Registered Nurse: Current RN license with the Texas Board of Nursing (TX BON)
e) Basic Life Support: Current Basic Life Support Certification from an American Heart Association or Red Cross approved program
f) Advanced Cardiac Life Support: Current Advanced Cardiac Life Support Certification from American Heart Association (as appropriate to specialty area)
g) Valid TX Driver's License
Work Experience:
a) 5 Years of Experience: General Hospitalist (Preferred)
Management Experience:
a) 2 Years of Experience: Acute Care: EC or Inpatient
Language:
a) Bilingual Skills: Spanish (Preferred)
Communication Skills:
a) Above average Verbal Communication (Heavy Public Contact)
