Department Hospital Care MgmtJob Summary
Under general professional supervision, performs social work/case management services including planning, organizing, treatment and inpatient discharge/disposition. Delivers social services in accordance with the approved plan of treatment and plan of care. Assists the physician and other healthcare members in understanding significant social and emotional factors related to the patient's health problems. This position is for 30 hr per week, Thursday, Friday, Saturday
Essential Functions: - Provides services which involves identifying client needs, planning for implementation or coordinating transitional care plans through resource acquisition, monitoring, advocacy and evaluation in collaboration with the RN Care Coordinators. Conducts psychosocial assessments that are individualized, objective and complete. Provides services appropriate for presenting problem. Consults, negotiates, and coordinates with internal and external resources to maximize patient outcomes. (20%)
- Applies basic knowledge of individual, group and family dynamics and treatment methods to assist client in achieving treatment goals. Demonstrates ability to develop effective interventions that are sensitive to socio-cultural factors. (20%)
- Participates in multidisciplinary patient care meetings (i.e. care conference & rounds) and utilizes information to assess and reassess care needs. Develops working knowledge of programs and eligibility of the programs. Demonstrates skill in obtaining appropriate health care resource/services and educating others. Participates in staff meetings, staff development and applies knowledge of groups and their tasks through appropriate participation. (20%)
- Monitors and evaluates effectiveness of services to consumers and modifies intervention plan as indicated. Professional activity complies with applicable laws. (15%)
- Assists clients to maximize adjustment to illness/disability/trauma by addressing biopsychosocial issues and enhancing problem solving and coping skills. Evaluates emotional components, impact of health issues and assists client to identify disruptive factors to establish treatment goals. (5%)
- Adheres to and complies with rules and regulations of the City, Agency, Department and Service. Attends all mandatory meetings and applies knowledge in daily practice i.e. safety and infection control and identified areas affecting practice. Practices in accordance with ethical principles established in NASW code of ethics and by Denver Health. (5%)
- Completes documentation of social work activities and interventions per established standards and guidelines. Applies written guidelines to assure work is accurate and complete. Prepares and submits data and information to CSW in a timely manner. Chart notes are legible, thorough, and timely and content reflects established qualitative standards. Documented activity reflects patient involvement in goal setting with written informed consent documented. (5%)
- Utilizes methods, techniques and approaches to the delivery of care that are consistent with the patient population and in conformance with the specific care plan developed for each patient. (5%)
- Develops knowledge base of risk factors for psychosocial functioning, wellness and health promotion and provides basic education to patient. (5%)

- 1-3 years Typically, one year of experience.

- Excellent interpersonal skills.
- Skilled in identifying problems/opportunities to improve care.
- Ability to help the patient and family to understand, accept, and follow medical recommendations.
- Ability to utilize resources, such as family or community agencies, to assist the patient resume life in the community.
- Ability to collect key information from the patient, family, and the community, as part of the social work assessment.
- Ability to communicate to the appropriate staff, information about the patient?s discharge plan.
