Compensation:
Pay Range: $30.70 - $49.90 / hour

Job Description:
To be accountable for daily supervision and coordination of case management department processes and work activities. To provide leadership and management of staff. To support department operations and enforce compliance with policies, procedures, and regulatory agency requirements.
1. Supervises department staff and oversees department resources in a manner that provides for effective patient care and proper utilization of resources.
2. Supervises staff including, but not limited to, staff selection, training, evaluating and disciplining. Supports staff development and competency inclusive of education and training.
3. Promotes continuous quality improvement and ensures processes are compliant with internal and external policies and regulatory standards. Reviews data and outcome trends and develops and monitors process improvement activities that drive departmental strategies and goals. Provides reports as appropriate.
4. Provides direct social work or RN care coordination services as required by staffing levels. Assists with patient case problem resolution. Maintains current working knowledge of resources needed to provide patients with transitional care.
5. Collaborates with internal and external resources to achieve goals.
6. Performs other duties as assigned.
