Home-based inpatient care is the wave of the future, expanding hospital capacity and supporting aging in place. Hospital at Home is an innovative mobile medical-surgical unit within the community. An RN2 with the Hospital at Home Unit: focuses on providing hospital-level care in the home setting; collaborates in establishing the acute hospitalization plan of care; coordinates and implements care delivery while anticipating patient care needs across the continuum. The Registered Nurse analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis; continually utilizes the nursing process to evaluate the patient's progress toward goals and applies appropriate interventions to ensure effective outcomes; and employs strategies to promote health and a safe, healthy environment. The responsibilities listed are a general overview of the position and additional duties may be assigned. As an RN2 with Vanderbilt's Hospital at Home Unit, you will be cross-trained into up to three roles: Field Nurse, Virtual Nurse, and Intake Nurse. This is a part-time position. The Intake Nurse (0645-1915) is based out of the adult hospital ED, screens potential patients, aids in the consenting process with the Hospital at Home attending physician, and coordinates transfers home. The Field Nurse (0645-1915) drives to each patient's home twice per day to provide the prescribed care. The Virtual Nurse (0645-1915 and 1845-0715) is based out of our command center in Crystal Terrace, monitors continuous vital signs via wearable technology, performs virtual medication administration, provides patient education, and addresses patient queries via telehealth. We offer a competitive hourly rate, evening/night/weekend shift differentials, and excellent benefits that begin day one of employment. We also have a relocation stipend for those wanting to relocate to the area. Hours: This is a Part-Time position. The candidate selected will work approximately 24 hours per week. The candidate selected will work 2x12-Hour Shifts per week. Department Summary: Many patients would rather be at home than in the hospital when they're sick. Our program provides the same high level of care you get in the hospital to the comfort of your home. You may be offered this service based on your condition, insurance plan and geography after you have been assessed in the hospital or emergency department. .
