Your Role on the Team Position Summary: The incumbent is expected to provide expert level psychosocial interventions and case management services to vulnerable older adults with complex medical, social, financial, community and behavioral problems. By engaging and educating families and caregivers involved in the older adult's care, the social worker works to optimize the social supports available to the individual to promote maximum functioning and quality of life. In collaboration with the primary care physician (PCP), the ACO team works closely together to assess the individual, develop and implement a care plan, evaluate and treat geriatric conditions, and provide longitudinal care management and coordination of care. As an independent practitioner of social work, the social worker evaluates psychosocial needs and applies social work interventions with a high degree of autonomy, exercising independent judgment and initiative in carrying out social work functions and in applying social work values and ethics to complex situations.
