Job Summary
The Transition Care Coordinator, Transitions and Post-Acute Care (TCC-TPAC) facilitates the collaborative interdisciplinary process of care management encompassing assessment, planning, facilitation, care coordination, evaluation appropriate to the scope of licensure. The TCC-TPAC advocates for options and services to meet the patient's and families comprehensive health needs through communication and available resources to promote quality cost-effective outcomes for the identified patient population. TCC-TPAC is accountable for outsourced patient's and collaborates effectively in order to meet patient care needs across the appropriately deemed continuum of care, provide family support, manage the length of stay, and promote efficient utilization of resources. The TCC-TPAC participates in quality improvement activities, exemplifies professionalism, and promotes a customer-friendly environment by utilizing ServiceFIRST behaviors in interactions with patients, families, and staff members.
Minimum Qualifications
Degree:
Bachelors required
Masters (Preferred)
Licenses & Certification:
a. Licensed to practice nursing in the state of TX
b. CCM or ACM or ANCC is required within 2 years of employment
c. Basic Life Support
Work Experience: Four (4) years of Experience as a Registered Nurse and 2 Years of Experience in Case Management, Quality Management, Population Health, Chronic Disease management
Management Experience: Two (2) years of Experience in Leadership/Clinical preferred.
Communication Skills: Above average Verbal Communication (Heavy Public Contact), Exceptional Verbal (Public Speaking), Writing/ Correspondence, Writing/ Reports
Languages:
Bilingual Skills: Spanish and Vietnamese Preferred
Proficiencies: MS Word, MS PowerPoint, MS Excel, MS Outlook, PC
