PHYSICIAN ASSISTANT - COMMUNITY LIVING CENTER / EXTENDED CARE

Summary

The Physician Assistant (PA) is accountable for providing Ambulatory Geriatric - long term rehabilitation - short term rehabilitation - respite - hospice - and palliative care as well as follow-up care for patients with an emphasis on maintenance and promotion of optimal wellness and the coordination of care across the continuum of health care - including end of life care.

Duties

• The Cincinnati VA Medical Center (CVAMC) is a two-division campus located in Cincinnati - OH and Fort Thomas - KY serving 15 counties in Southwest OH - Northern Kentucky - and Southeast Indiana with 6 Community Based Outpatient Clinics located in Bellevue - KY
• Florence - KY
• Lawrenceburg - IN
• Hamilton - OH
• Clermont County - OH - and Georgetown - OH
• The CVAMC also supports two off main campus locations in the Cincinnati area via an Eye Center and Mental Health Outreach Community Division
• The major duties of the Physician Assistant - Community Living Center / Extended Care include - but are not limited to: Performs and documents histories and physical examinations - providing and coordinating comprehensive care for assigned patients in his/her assigned care setting
• Documents progress notes and summaries in the patient's medical record - ordering of therapeutic regimens as well as various diagnostic tests - documenting a discharge summary
• Provides education and counseling to patients and families in preventative care - medical conditions - and the use of prescribed treatments and drugs
• Provides and coordinates medical care for assigned patients in any care setting - including establishing diagnoses - formulating and implementing care plans - and providing follow-up care
• Accountable for adhering to the established policies - standards of care and practice and achieving the goals for the Clinical Guidelines and Prevention Indicators
• Utilizes advanced skills in assessment - problem identification and clarification - treatment - planning and implementation of the patient's treatment plan
• Collaborates with other members of the health care team to design and/or continuously improve the quality of care delivered to veterans
• Work Schedule: Full-time
