YOUR ROLE As the Program Manager of Chronic and Complex Patient Management (CCPM), you will oversee a team dedicated to providing longitudinal care management for patients with complex and chronic conditions, including kidney disease, diabetes, heart failure, and other comorbidities. You will serve as a collaborative and supportive leader responsible for the day-to-day operations of your care management team, focusing on proactive, patient-centered care to manage chronic diseases effectively. By leveraging expertise and experience in managing patients with chronic and complex needs, you will ensure care pathways are followed to minimize complications, prevent hospital admissions, and enhance patients' quality of life. You will also manage CCPM program performance and provide continuous training and development to ensure Evergreen’s key performance indicators (KPIs) and performance metrics meet or exceed expectations.

Compensation The pay range for this role is $113,000 to $120,000 annually. Exact pay is determined based on experience, education, demand for the role, and other role-specific factors.
