Under minimal supervision. The RN-Wound Care Specialist provides acute and rehabilitative care for patients with certain conditions of the gastrointestinal, genitourinary, and integumentary systems. Assists in developing and implementing initial training, future training and care team development programs related to wound care assessment, management, and documentation for the assigned area(s) Conducts ongoing education for team and is responsible for partnering with nurse educators to assure competency of the professional nurse and unlicensed personnel in the assigned area(s). Evidence of competency is reviewed and documented based on the most recent evidence and best practices. Leads the department's evidence-based wound care orientation program and serves as a clinician, collaborator, consultant, and educator. Adheres to the American Nurses Association (ANA) Nursing Administration Scope and Standards of Practice to provide leadership with the nursing process. Utilize and support the Theory of Human Caring and Caritas as a part of his or her daily practice. Essential Functions:
- Assessment -Develops, maintains, and evaluates healthcare consumer and care team processes to support the practice of nursing and delivery of healthcare consumer care.* Identifies elements specific to nursing-sensitive indicators appropriate for the area* Identifies elements specific to needs for additional resources to support the patient to and within the acute care setting. * Utilizes current research findings, current practice guidelines, and standards to modify and improve pressure injury data collection processes.* Collaborates with appropriate departments to utilize assessment data and quality, safety, cost-effectiveness data to deliver measurable outcomes as criteria for success* Establish and meet practice area, department, and agency goals that align with the organizational and nursing annual plans.* Coordinates, implements, monitors and evaluates division/area-level customer service plan. (20%)
- Diagnosis - Develops, maintains, and evaluates an environment that empowers and supports the professional nurse in analysis and assessment data and in decisions to determine relevant problems and outcomes. * Assists and supports care team in developing and maintaining problem/diagnosis competencies* Facilitates inter-professional collaboration.* Assures a system that facilitates a healthcare consumer-centered plan of care and desired outcomes* Participates in nursing and other organizational-wide committees.* Collaborates proactively with nursing leadership to maximize area effectiveness through innovative recommendations and Lean Initiatives. * Participates and promotes a climate of clinical inquiry (20%)
- Outcomes Identification - Develops, maintains, and evaluates information systems and processes that promote desired healthcare consumer, professional, and organizational outcomes. (20%)
- Plan - Develops, maintains, and evaluates organizational systems to facilitate planning for the delivery of care. * Fosters inter-professional collaboration and planning that focuses on healthcare consumers and families.* Promotes the integration and applicable organizational theories, evidence-based nursing, and related research findings and practice standards and guidelines into the planning process. * Participates in the development and implementation of a system to promote the rights and ethical treatment of the healthcare consumer and inter-professional teams. * Develops strategies that address identified issues, problems, or trends.* Reviews and evaluates plans for appropriate utilization of care team at all levels of practice criteria the provisions of the Colorado state nurse practice act and professional standards of practice (20%)
- Implementation - Develops, maintains, and evaluates organizational systems that support the implementation of plans and delivery of care across the system.* Facilitates care team participation in decision-making regarding the creative development and implementation of organizational systems, and the resources that assure that interventions are safe, effective, efficient, age-appropriate, and culturally sensitive.* Reviews literature on implementing best practices and benchmarking.* Utilizes evidence-based interventions specific to the problem or issue.* Develops and implements strategic plans with attainable goals for each area of responsibility in collaboration with nursing administration and other departments. (10%)
- Evaluation - Evaluates the division of nursing strategic patient and safety quality plans and its progress concerning the attainment of outcomes.* Advocates for and supports innovative processes of participative decision-making.* Ensures educational opportunities for the care team specific to the population served, professional practice, available technologies, or required skills to enhance quality in healthcare delivery.* Fosters participation and recognition of the care team in internal and external, formal, and informal organizational evaluation committees, teams, and task forces.* Ensures sufficient resources to provide for critical assessment and evaluation of desired outcomes, including allocation of individual care team time for meaningful involvement in shared leadership. * Uses the results of the analysis to make or recommend process or structural changes including policy, procedures, guidelines, and protocols. * Utilizes data generated from outcomes research to develop innovative changes in care delivery. (10%)
Education:
- Bachelor's Degree Required
- Master's Degree Nursing or related field
Work Experience:
- 1-3 years in nursing with direct patient care Required
Licenses:
- BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association or American Red Cross Required and
- RN-Registered Nurse - DORA - Department of Regulatory Agencies Required
- WCC-Wound Care Certification RN - WOCNCB - Wound, Ostomy and Continence Nursing Certification Board
Knowledge, Skills and Abilities:
- Knowledge of regulatory standards and compliance requirements.
- Skilled in decision making regarding the appropriateness of care review, assist with research design planning, and collaborating with the Nursing Director on the skin and wound care-related quality improvement reports.
- Ability to work independently as well as collaboratively with other internal and external team members to develop and ensure timelines are met.
- Ability to act as a resource to the care team for clinical questions. Able to effectively recruit, develop, and retain the care team.
- Proficient in Microsoft Office Suite.
- Epic Documentation competence preferred.
