 Infusion Home Health RN (HOME HEALTH NURSE 2) Job Summary Registered Nurses in the Home Infusion department at UC Davis Health provide infusion care to adult and pediatric patients with a variety of health needs. The HH RN 2 identifies and implements nursing interventions and evaluates results for a wide variety of patient populations, directs and acts as resource to other members of the nursing team as assigned and promotes compliance and support for policies. The primary responsibilities for this position consist of providing total, comprehensive, continuous and patient centered care which includes: assessing, documenting, planning, implementing, teaching, developing, and evaluating a comprehensive nursing plan of care; delivering direct patient care; ensuring patient satisfaction; training and delegating duties; and adhering to University policies and procedures as well as federal, state and local regulations. Autonomous nursing care is the ability of a nurse to assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice autonomously consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care. Department Overview UCDH Home Infusion is a new department aimed at bringing advanced infusion care to patients in the comfort of their homes. Our purpose and values are centered from a patient-driven perspective and in partnership with physicians and other providers. The department prides itself on its delivery of user-friendly Home Infusion care for all patients, caregivers, and members of the health care team.The role of the Home Infusion RN is to provide exemplary infusion care to patients in their homes. The position will require travel to patient's homes and administration of intravenous and subcutaneous infusions to support patients' health goals. Home Infusion RNs are experts in vascular access care and employ a wide range of techniques to ensure patients receive an exceptional care experience. Home Infusion RNs provide standard and progressive infusion therapies such as immunotherapies, enzymes, and other specialty medications and must have the knowledge and skills necessary to respond to infusion reactions autonomously in the home. Department Specific Job Scope UC Davis Health is a nationally recognized leader in the provision of high-quality, patient-centered care, based in the Greater Sacramento area of Northern California. UC Davis Health has a Comprehensive Cancer Center, internationally recognized MIND Institute, and a nationally ranked pediatric hospital. The UC Davis acute care hospital has 646 beds and an ambulatory care service area spanning north to the Oregon border, east to Nevada, and down the Central Valley to Fresno, California, covering 65,000 square miles across 33 counties. The Department of Pharmacy is a dynamic, integral, and critical part of patient care provided by UC Davis Health. As a department of greater than 600 professional, technical, and administrative support personnel, we focus on providing patient-centered medication management that optimizes outcomes through practice, education, research, and leadership through the continuum of care. We also have the largest pharmacy residency program in the country with 46 total residents in 13 different programs. The Department of Pharmacy offers comprehensive pharmaceutical services (unit dose, IV admixtures, clinical services, infusion pumps, and teaching) throughout the health system complex 24 hours a day, 7 days a week. The pharmacy services encompass inpatient pharmacy services as well as an expansive footprint in the ambulatory care space where we have clinical pharmacists embedded within our clinics and specialty care locations. Our operational teams support the health system enterprise with three inpatient pharmacies, five outpatient pharmacies, two infusion pharmacies, a home infusion service line, and the full complement of pharmacy purchasing, stores, and distribution.
