Job Summary
The Clinical Nurse Care Manager (CNCM II) position supports the healthcare team in transitional care by being a liaison between patients, their families, doctors, and health care providers. CNCM II facilitates transitional care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction. In addition, the CNCM II helps to drive change by identifying areas where performance improvement is needed (e.g., day to day workflow, process improvements, patient satisfaction). CNCM II is accountable for patients caseload and plans effectively in order to meet patient needs across the appropriately deemed continuum of care, provide family support, manage the length of stay, and promote efficient utilization of resources. CNCM II advocates for patients and coordinates patients care and work with other health care professionals to determine patients goals and create both short term and long term plans. CNCM II participates in quality improvement activities, exemplifies professionalism, and promotes a customer-friendly environment by utilizing ServiceFIRST behaviors in interactions with patients, families, and staff members.
Minimum Qualifications
Degrees: Graduation from an accredited school of Nursing with a Bachelors degree in Nursing required, MSN - preferred.
License & Certifications:
Registered Nurse; Licensed to practice nursing in the State of Texas.
Basic Life Support from American Heart Association (AHA) or Red Cross approved program.
Certified Case Manager (CCM) or Accredited Case Manager (ACM)
or American Nurses Credentialing Center (ANCC)
Work Experience:
Four (4) years experience as RN appropriate area assigned required;
Two (2) years experience in Case Management, Quality Management, Coding required
Management Experience: Two (2) years leadership/clinical experience preferred.
Communication Skills:
Above Average Verbal (Heavy Public Contact), Exceptional Verbal (e.g., Public Speaking), Writing/ Correspondence, Writing/ Reports
Proficiencies: MS Word, P.C.
