Job DescriptionLead Clinician will demonstrate strong communication/interpersonal skills, critical thinking and problem-solving ability, organizational and time management skills, Knowledge of healthcare policies and procedures. A strong basis of knowledge of Social Work Theory and Practice. Assess and fosters patient, caregiver, and/or family coping, resiliency, and adaptability skills. Assess patient, caregiver, and/or family members understanding of treatment options, diagnosis, side effects, outcomes etc. Promote and facilitate communication between patients, caregiver(s), and/or family. Utilize patient self-assessed distress screening tool to identify barriers of care, emotional concerns, and levels of distress. Review and follow-up with patients/families. Assess, provides interventions where appropriate, and facilitates referrals for issues related to abuse/neglect, substance abuse, depression, anxiety, and other mental health disorders. Collaborates with community agencies on patient services needs and programmatic efforts. Demonstrates knowledge of psychiatric specific and community resources. Make referrals as appropriate. Provide psychotherapeutic counseling for patients and families Facilitate goals of the psychiatric plan of care Facilitate support groups including psychoeducation, experiential, and psychotherapeutic groups. Provides preventive, protective, or supportive services to patients who are in or threatened by situations detrimental to their well-being. Collaborates with the health care team regarding in-hospital needs and post-hospital care. Maintains current with available psycho-social services programs/services and provides relevant information to patients, families, staff, and community organizations. Coordinates and maintains a viable psychosocial discharge planning program for the assigned service area to identify patient needs and plan for appropriate after-care services. Ensures continuity of care through comprehensive and appropriate discharge planning with patient, family, and/or significant others. Documents appropriate data from psycho-social assessments, individual contacts, family interventions and discharge planning according to department policy and procedures and State, Federal, and Joint Commission stipulated requirements. Develops a social service network with city, state or other governmental services that serve the needs of the assigned patient population, to assist patients in obtaining such services as, food stamps and other financial supports and assistance in obtaining transitional housing. Evaluates psycho-social services program objectives and results and participates in program planning and improvement in accordance with the mission of the hospital and department. Maintains confidentiality in accordance with hospital and departmental policy and professional standards. May accompany patients to obtain benefits, entitlement, or other services.
Track and report on unit QI metrics Maintain Social Determinants of Health (SDOH) Supervise interns Supervise the completion of daily 1:1 patient session's Project lead for all unit-based projects Participate in daily nursing/unit huddles PACT participation Active role in Unit Based Shared Governance (UBSG) Active participation in unit leadership meetings Facilitate and document daily transitions of care meeting Facilitate treatment team meeting in a timely, efficient format Participate in unit education/ yearly competencies Group facilitator and content expert Supervision of LSW to obtain LCSW
