Job DescriptionThe Opportunity: If you are a dynamic, motivated individual who is driven to create healthier communities through excellent patient care, Saint Luke's has a fantastic opportunity for a PRN Social Worker in our in-patient environment. Saint Luke's is a faith-based, not-for-profit, aligned health system committed to providing the highest levels of excellence in compassionate health care and health-related services. For over 135 years, we've been dedicated to enhancing the physical, mental, and spiritual health of the diverse communities we serve. Shift: Part-time, day shift, 8:00 a.m. - 4:30 p.m., 3 days per week. There is a weekend and on-call requirement as well. Responsible and accountable for assessing the psychological social emotional cultural or financial needs that impact patient's health and recovery. To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning. Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner. Participates in program planning and development to meet the needs of customers within the health care system and community.
EXPERIENCE: Prefer prior experience in a healthcare setting (i.e., hospital, home health or nursing home) but not required. SPECIAL SKILLS: Excellent communication, interpersonal, organizational and presentation skills. OTHER: Computer skills, Basic typing skills EDUCATION: Required:
