HIM Rep II

Job Summary:

HIM Representative II is responsible for clerical duties with in the Health Information Management department.

Responsibilities:

• Organizes, maintains, and evaluates medical records (electronic or paper)
• Responds to requests to review medical records/and or retrieve missing documentation
• Researches any significant name or medical record changes and notifies management.
• Ability to analyze complex information and use problem solving skills to determine appropriate solutions and necessary merges to protect the integrity of the EMPI.
• Corresponds with Network hospitals regarding duplicate number identification and merging.
• When necessary, merges duplicate medical record numbers in the MPI in a timely fashion.
• Reviews, collates, and submits SPARCS edits and communicates with Revenue Cycle departments to resolve SPARCS rejections.
• Accesses various systems and maintains departmental dashboards.
• Collects the necessary documents, data, and information for the correct completion of birth certificates and/or other types of vital records.
• Pull all medical record requested by hospital departments for patient care purposes.
• File all medical record charts in permanent file according to the facilities terminal filing system. All charts will be filed by the end of each shift.
• Research all loose reports and locate corresponding medical record for proper filing.
• Pull charts for physicians as they request their records for completion.
• Answer all phone calls in a timely manner.
• Assists customers as needed with courtesy and respect.
• Ensure the privacy and security of patient information.
• Other duties as assigned

Experience:

1 to 2 years HIM experience preferred. Experience working in a health care setting.

Education:

High School diploma or equivalent required, associate or bachelors preferred
