Social Worker (Program Coordinator)

The Social Worker Program Coordinator is independently licensed at the Advance Practice level to the Health Care for Homeless Veterans (HCHV) Contracted Residential Services (CRS). The HCHV CRS Liaison is responsible for the monitoring and coordination of services provided by community-based programs funded under the VA emergency housing contracts.

Duties

• The CRS [Contract Residential Services] liaison provides supportive services and case management to Veterans residing in Contract Emergency Residential Housing facilities by monitoring clinical care and assess the compliance of the programs receiving payments - as described in the recipient's contract
• The CRS liaison provides oversight of the Contract Residential Services facility
• This includes - but is not limited to - verification of Veteran's status and eligibility for participation in programming - maintaining admission and discharge dates of participants for billing purposes - reviewing and approving/disapproving readmissions to and extensions of care
• The CRS liaison serves as the Contracting Officer Representative (COR) for the Contract Emergency Residential Housing Program - upon completion of required training and certification
• The incumbent coordinates care and acts as an advocate on behalf of Veterans to ensure comprehensive service delivery
• Utilizes various treatment modalities of individual and group work - casework - community organization and community outreach activities to alleviate or remove problems that interfere with the Veteran's ability to achieve independence
• Provide a wide range of services - including assessments - referrals - short-term case management - advocacy - and service coordination
• Provide a full range of social work services within commonly accepted standards of social work practice which includes case work - individual - family and group counseling - discharge planning - case management and other services as appropriate
• Evaluate and assess Veterans at risk of danger to self and/or others and utilize the appropriate emergency channels within the VA system or local community dispatch agents to address the crisis
• Perform timely accountability functions: 1) document all contacts with Veterans in the electronic health record charts
• 2) complete statistical and other reports in accordance with VA CERS and respective professional discipline service policies
• TRAVEL REQUIRED: Occasional by car to travel to Veterans Contract Residential Services Programs and or in the community to assist Veterans with housing as needed and also to conduct site visit of contract agencies
• Work Schedule: Monday-Friday 8:00 am to 4:30 pm Compressed/Flexible: Available Telework: Authorized for ad-hoc per GLA policy after first 90 days of employment
