Job DescriptionAtlantic Health is seeking a dynamic and motivated individual, to join the energetic and innovative team of healthcare professionals in their Atlantic Visiting Nurse Department as a Full-Time Physical Therapist to work 8AM-4PM including one weekend every four to six weeks, the Essex County area. The ideal candidate will evaluate and treat patients in the patient's home and be a part of a collaborative team of health providers. Atlantic Visiting Nurse provides personalized care for you or a loved one to regain health, stay out of the hospital, and remain living independently at home. Whether or not it is managing life with a chronic illness, living with a disability, returning home from the hospital or rehabilitation facility, or need assistance with end-of-life care, we are available to you in six counties throughout New Jersey: Morris, Somerset, Essex, Sussex, Union, and Warren. Responsible for providing a broad range of physical therapy services in an interdisciplinary setting, including assessment, treatment planning, and therapeutic interventions in accordance with the position's qualifications, professional practices, and ethical standards. Ensures and provides development, quality improvement, problem-solving, and productivity enhancement in a flexible interdisciplinary manner. QualificationsEssential Functions/Responsibilities:
