Job Summary   Kaiser Permanente-Washington Permanente Medical Group is seeking a full-time 1.0 FTE Primary Care Physician Assistant to join our Quality, Documentation &#38; Reporting Team.       The VC3 (Virtual Care for Chronic Conditions) Clinician will work to ensure quality care for identified WPMG patients utilizing virtual encounters to address conditions and close care gaps in a patient centered manner. Whenever possible, they will preview, update, and modify the health record in HealthConnect to optimize the chart for their virtual encounters and perform PC3 (Patient Centered Charting and Communication) chart reviews between virtual visits. The work completed by the VC3 Clinicians will be monitored through proficiency metrics and chart audits to ensure quality of documentation. VC3 Clinicians will report to the Assistant Medical Director of Documentation and Reporting.   Principal Duties &#38; Responsibilities            * Conduct on-demand virtual visits for adult members in conjunction with existing Specialty Care encounters or when the patient is onsite for other care needs. VC3 Clinicians will be expected to be available during normal clinic hours Monday through Friday. Between VC3 virtual visits, chart reviews will be conducted to ensure completeness and accuracy of the medical record. These will be done according to PC3 Clinician Reviewer standards.    * Dedicated FTE must be an increase over their currently benefitted FTE and may not exceed a total FTE of 1.0. Any request to decrease clinical FTE to take on this role would require approval by Primary Care leadership (AMD &#38; DMD).    * Be proficient with care gap closure and provide accurate and complete documentation of disease state management to address conditions.    * Provide expert integrated care and diagnostic services in the virtual care setting. Coordinate care throughout KPWA remotely ensuring remote standards of care    * Work remotely in compliance with all WPMG &#38; KPWA remote work policies. Clinicians may work in a KP facility if a remote connection is not available or at their discretion provided no impact to in-person operations.    * Work closely with Primary Care, Specialty Care, and THMC to improve care coordination for our members.    * Perform PC3 chart reviews between virtual visits according to PC3 standards by reviewing multiple sections of the medical record and updating appropriately, using clinical judgement to add, resolve, delete chronic conditions from the Problem List and/or MCR Best Practice Advisory (BPA), consolidate/refine diagnoses as needed, updating overview notes with relevant supporting clinical information, and prioritizing the Problem List according to PC3 guidelines.    * Maintain flexibility in staffing to ensure coverage for VC3 department, including working to arrange vacation/PTO to coordinate with other VC3 providers to maintain adequate coverage.    * Maintain an appropriate work from home environment including professional background, quiet space from which to work without distractions, and privacy to ensure patient confidentiality.    * Work effectively in an inter-disciplinary team collaborating with care teams to explore patient health conditions and diagnose health issues, while ensuring positive, effective patient experiences. They will maintain knowledge of cultural competency, particularly related to how variables related to socioeconomic status, race, ethnicity, and culture impact physical and emotional health.    * Analyze reports, test results, medical records, and examinations to diagnose conditions and propose treatment options as well as order appropriate laboratory and radiology tests, medications, or other treatments/referrals as needed for patient health issues and documented medical conditions.    * Review/update the medical record including current medications, allergies, medical/surgery history, etc. to ensure accuracy of information and provide quality patient care.    * Provide advice to patients for lifestyle and diet changes that may improve their health or help to treat the health issue they are experiencing.    * Adhere to departmental policies, procedures and objectives, ongoing quality improvement objectives and safety, environmental, and infection control standards.    * Maintain patient confidentiality and comply with all federal and state health information privacy laws.    * Record complete, timely electronic medical records that meet documentation and coding standards.    * Be available to work in at least half day increments (0.1 FTE). Compensation and   Benefits            The entire salary range for the clinician position is $136,151 to $163,708 and will be prorated based on FTE. Placement of new hires will be in the range of $136,151 to $155,522 based on factors such as education, training or experience.
