POSITION OVERVIEW:  The Nocturnist Hospitalist Physician responds to admission requests and assumes care at such time that patients are transferred out of the Emergency Department or directly admitted from an outside facility as specified by each SCHS hospitals medical staff bylaws. The Nocturnist Hospitalist Physician (night shift) provides medical care as the attending of record for adult inpatients (minimum age 18) presenting with primary internal medicine pathology and provides their services twenty- four (24) hours daily all days of the year. Work schedule:  Shifts are generally ten to eleven (10-11) hours in duration and Physician will work minimum of 118 shifts per year for a .9 FTE Nocturnist. There is the potential to workday or swing shift as well. Option of 7-day rotation schedule or option of flex scheduling. Call expectation:  Back-up call during high census times may be requested. Physician Practice Location:  Physician will render the Professional Services at such SCHS Facilities as may be directed by SCMG, and at such other facilities and locations as may be established by SCHS or SCMG from time to time. It is anticipated that the primary work locations will be as follows: Practice Locations: St. Charles Hospital Bend [2500 NE Neff Road, Bend, OR]  St. Charles Hospital Redmond [ 1253 Northwest Canal Boulevard, Redmond, OR] ESSENTIAL FUNCTIONS AND DUTIES:  (May perform additional duties of similar complexity within SCHS as required or assigned) General Duties: Provide 24-hour coverage of all Medicine patients on the Bend campus, both in an admitting role as well as a consultative role, in accordance with existing Co-Management Agreements. Provide 24-hour coverage of all Medicine and Critical Care patients at the Redmond campus, both in an admitting as well as consultative role, in accordance with existing Co-Management Agreements. When requested by the primary service, serve as a consultant for all inpatient psychiatric patients in PES or Sageview. If medically complex, serve as a consultant for patients in Inpatient Rehabilitation. Share in equitable distribution of work between all hospitalist physicians on shift. Maintain credentialing and privileges to perform critical care procedures, and ability to rotate through the St. Charles-Redmond hospital on an equitable basis with others in the position. May also transfer patients requiring a higher level of medical or sub-specialist care from the St. Charles- Redmond hospital to the SCMC-Bend hospital or another facility at their discretion and that of the accepting physician at the receiving facility. Maintain familiarity with the SCMG organization structure, communication systems, billing process, and completing billing and coding documentation within time assigned by SCMG administration. Obtain medical knowledge and current skills through regular continuing medical education (CME) activity. Abide by all medical staff bylaws, rules and regulations. Share information both when requested and when anticipating someone might have a need. Maintain punctual presence for assigned workdays, in compliance with the SCHS policy on attendance Keep medical knowledge and skills current through regular CME activity. To attest to having obtained 60 hours of Category 1 continuing medical education (CME) relevant to the practitioners current medical practice over the prior two-year period. Participation in Rapid Response Supervision may be requested from time to time. Specific Duties: Document all patient care within the Electronic Medical Record (EMR) and complete all necessary tasks in compliance with health system policies and regulatory agencies to ensure timely and accurate billing. Be familiar with the SCHS organization  structure. Participate in SCMG meetings and assist in policy formation for SCMG and Physicians  practice. Keep medical knowledge and skills current through regular CME activity. Must attest to having adequate (CME) to meet maintenance of board certification and compliance with MSO rules and continuing privileges without restriction. This service line includes approximately 60 providers representing the full spectrum of neuromusculoskeletal care across inpatient, outpatient, ambulatory surgery centers and clinics. The team will collaborate extensively with the Bend-hospital based acute surgery and trauma team in the development, refinement and collaboration of a robust trauma program designed to meet the growing needs of the Central Oregon community. POSITION OVERVIEW:  Orthopedic Surgeons are responsible for providing medical, surgical, and postoperative management of patients having Orthopedic surgery. Each orthopedic surgeon will have a subspecialty focus to include one of the following: Sports Medicine, Total Joints, Foot &#38; Ankle, Hand, or Trauma. In some cases, surgeons will split their practice to include one or more sub-specialty area for their practice. Orthopedic surgeons will work in collaboration with other medical specialties to assess and manage patients following established standards and practices. This position will primarily work in the Bend and Redmond hospitals, Cascade SurgiCenter and Bend Surgery Center performing elective and acute surgeries. Orthopedic Surgeons also see patients in a clinical setting and work collaboratively with APPs in the care of patients. It is up to the physician to determine the appropriate site of care for each patient based on the patients medical presentation. Work schedule:  Physician will have roughly 32 patient facing hours per week as a 1.0FTE which is prorated proportionate to their FTE. This will be a blend of hospital rounding, procedure block time and scheduled clinic visits including charting of patients and other necessary duties, recognizing that the minimum requirement may fluctuate up and down based on direct patient care. Call expectation:  Surgeon is expected to participate in the orthopedic hand call rotation. Ortho hand call provides coverage from elbow to fingertips. If qualified, the physician may also participate in the Orthopedic Trauma room coverage. POSITION OVERVIEW:  Advanced Practice Providers (APPs) under the general supervision of the Clinical Division Director and direct supervision of a designated physician, provides medical and appropriate outpatient care to patients. Such care includes evaluation, diagnosis, treatment, and documentation of each care encounter. APPs will assist GI MDs by providing inpatient consults and rounding on department patients. Coordinates medical, ancillary, nursing, and other services. Monitor patients&#39; on-going care plan and refer them to other departments based on their assessment of patients&#39; progress and condition. Work schedule:  APP will work a minimum of 40 hours per week with a minimum of 36 hours patient contact time. Scheduled days may vary depending on clinic operating days and hours. Call expectation:   APP   will   be   expected   to   participate   in   the   clinic   call   rotation   shared   equally   by   all   providers in   the   designated   call   group   to   include   evenings,   weekends,   and   holidays.   This   is   scheduled   in   coordination with other providers in the call group. ESSENTIAL FUNCTIONS AND DUTIES: Clinical  Expectations: The Gastroenterology APP will work primarily in an outpatient setting with both adult and pediatric patients, generally 16 years and older. Obtains pertinent medical history and appropriate physical  evaluations. Orders and interprets pertinent lab and/or x-ray studies and conveys the information obtained to the supervising Physician in a meaningful way. Performs and documents preprocedural and preadmission histories and physicals as well as discharge summaries. Facilitates referrals to community health facilities, agencies, and  resources. Recognizes medical emergencies and initiates evaluative and treatment procedures necessary in life-threatening situations and immediately reports to supervising physicians. Interpret and report on diagnostic tests in a timely manner in compliance with St Charles defined Standards. This should not exceed 72 hours for Urgent and 7 days for Routine diagnostics. Support the delivery, measurement and improvement of quality health measures as identified by SCMG. Consults with the physicians, nursing staff, and technicians to evaluate standards of patient care provided and makes recommendations for improvement. Completes clinical records in a timely manner fostering good communication between the health care providers and providing for continuity of care and coordination of services. Prescribes medications within the scope of their practice and Oregon  law. Performs procedures within the scope of their practice and the law, under the direction of the supervising physician. Performs other duties as  assigned. Delivers customer service and/or patient care in a manner that promotes goodwill, is timely, efficient, and accurate. Administrative   Expectations: Document all patient care within the Electronic Medical Record (EMR) and complete all necessary tasks in compliance with health system policies and regulatory agencies to ensure timely and accurate billing. Complete annual CME as required to maintain license and appropriate Board  certification. Complete in a timely manner assigned Computer Based Learning  modules. LICENSURE/CERTIFICATION/REGISTRATION Required: Unrestricted Physician Assistant and/or Nurse Practitioner licensure in the state of  Oregon. Physician Assistant requires NCCPA certification. Nurse Practitioner requires AANC, NCC or AANP, AMCB certification. If Physician Assistant, ability to establish and maintain a Practice Agreement with a Supervising Physician through OMB. Unrestricted Federal DEA number registered in  Oregon. Unrestricted ability to participate in Medicare / Medicaid, or other federal or state governmental health care programs. Valid drivers license and the ability to meet SCHS driving  requirements. Current BLS certification  required. If provider will practice sedation, moderate or deep, ACLS certification is also  required. Interventional  Cardiology will work in collaboration with other medical specialties to assess and manage patients following established standards and practices. Work schedule:  Full time Physician duties include a blend of procedures, scheduled clinic visits and interpreting cardiac diagnostic studies. This includes clinical documentation and all other necessary duties. 4 days per week. Each provider has a day off each clinic week. Each week consists of a combination of: Bend outpatient clinic: clinic hours between 8am - 5:00 pm, with a minimum of 12 appointment slots daily. This position will primarily work in the Bend hospital, Cascade SurgiCenter and Bend Surgery Center performing elective and acute surgeries.Surgeons also see patients in a clinical setting and work collaboratively with APPs in the care of patients. It is up to the physician to determine the appropriate site of care for each patient based on the patients medical presentation. Work schedule:  Physician will have roughly 32 patient facing hours per week as a 1.0FTE which is prorated proportionate to their FTE. Other duties may include performing basic physical exams such as sports physicals and occupational exams. The ability to work efficiently and refer appropriately is crucial. The ability to stabilize patients who present needing a higher level of service while awaiting transport is required. The Urgent Care functions as a unified regional department and shifts will be rotated among current and future locations. Laboratory services and X-ray radiology services are typically integrated into the department. Providers should have emergency department, urgent care, or primary care experience with substantial acute care exposure. The prospective candidate should be comfortable providing care for general medical, neurologic and neurosurgical patients, cardiac and cardiac surgical patients and surgical sub-specialty patients. The Bend ICU is staffed 24/7 with a board-certified critical care physician.   Every   patient   in   the   ICU   has   a   physician   responsible for   their   care and  has access to variety of  specialized   physicians  for  consult   on   a   patient&#39;s condition.  POSITION OVERVIEW:  The   Advanced   Practice   Providers   (APPs)   work   as   part   of   the Critical   Care   team.   They   work under the direction and supervision of the attending intensivist and facilitate care of ICU patients from admission to discharge, in collaboration with the attending physician.  The APP will discuss and update the Intensivist on duty with significant changes in the patients condition, new test results, and will attend Team rounds daily. New admissions will be discussed collaboratively with the Intensivist to develop a Plan of Care.  This is  NOT  an independent practice model for APPs and at all times a patients plan of care and actual care should reflect the informed judgment of the Attending Intensivist. Each APP should have   a   strong   foundation   in   evidenced   based   practice   and   ensuring   an   environment   of   safe,   quality   care.   They should also   demonstrate   an advanced level of medical knowledge, clinical/technical competence, solid clinical decision making, critical thinking skills, professional communication skills, timely and complete documentation, and are responsible for personal and professional development with competency  validation. APPs provide quality patient and family centered care as a member of the health care team in accordance with SCHS mission, philosophy, policies, and procedures; They provide specialty medical care within defined scope of practice under the supervision of a licensed physician, are responsible   for   daily   patient   flow   in  the  ICU   setting, and use critical   skills   to   make   appropriate   patient   health   assessments   and   that should align with the physicians plan of care. APPs do not directly manage any other caregivers. Work schedule:  APP will work at least one hundred fifty-six (156) shifts per reconciliation year for a full-time exempt employee and will be prorated for any FTE that is established below a full-time status. Provider Practice Location:  Provider will render the Professional Services at such SCHS Facilities as may be directed by St. Charles Health System, and at such other facilities and locations as may be established by SCHS from time to time.
